Assessment Service Agreement

Session:
The Initial Vocal Assessment is a 60-minute private session (Zoom or in-person when available). It includes an evaluation of posture, breathing mechanics, resonance patterns, vocal tension, projection habits, and overall vocal efficiency.

What’s Included:
• 60-minute personalized vocal assessment
• Breathing + posture analysis
• Resonance placement test and tension mapping
• Customized warmup recommendations
• Hydration and recovery guidelines
• Summary of your vocal strengths and focus areas

Fee:
The assessment fee is a flat $99. This may be applied toward a future coaching package if desired.

Payment Terms:
• Full payment is due upon assessment booking
• Rescheduling allowed with at least 24 hours’ notice
• Payments are non-refundable once submitted

Confidentiality:
All sessions remain private. No recordings are made without explicit consent.

Legal:
Nevada law governs this agreement. Electronic signatures are legally binding under U.S. E-SIGN and UETA.
